Why Choose FPA?

Family Practice Associates was founded by Dr. Jeffrey Foxx in 1983 and has grown to one of the largest family practice groups in Central Kentucky. Today FPA  is a group of primary care providers who are dedicated to giving family-centered care from birth to later years. FPA has 18 primary providers including: physicians, nurse practitioners, and a marriage and family therapist. With a professionally trained staff of CMA’s, receptionists, lab techs and others, FPA provides award winning care that has been recognized as a Patient-Centered Medical Home (PCMH) through the National Commission on Quality Assurance.

 

Patient-centered medical care is a model of care emphasizing coordination and communication. Its goals are to improve quality, enhance preventive services and to provide comprehensive chronic disease management. Introduced in 1967 by the American Academy of Pediatrics, the concept has broadened to include such descriptive phrases as accessible, continuous, comprehensive, family-oriented, coordinated, compassionate, and culturally effective. The PCMH model organizes care around patients, working in teams, and coordinating and tracking care over time.

 

Practices that are PCMHs become the “first contact” for patients and provide continuous and comprehensive care. Under the direction of the PCP, FPA’s patient care teams collectively take responsibility for on-going care of patients. The practice uses the “whole-person” approach which means patients actively participate in decision-making and give feedback to their provider about the care they are receiving.

 

Additionally, providers that are designated as PCMHs are experts at coordinating and integrating care through all levels of the health care delivery system including specialists, diagnostic services, hospitalization, home health, and long-term care. The model also takes into account the need for families to have accessible healthcare including same-day scheduling and extended hours. FPA is working diligently to offer Central Kentucky families enhanced access to medical care at its Hamburg Pavilion location and online at www.fpalex.com.

 

FPA Patients benefit from the FPA being an early adopter of health information technology. In addition to FPA having an improved practice website, the practice has integrated a patient portal and electronic health records (EHR) into the services it provides to families. All of these forms of technology help patients receive optimal patient care, communication, and patient education.

Rajeana M. Conway, MD

Rajeana M. Conway, MD

Dr. Rajeana Conway is originally from Maysville, Kentucky. She is married with two daughters. She enjoys spending time with her family, going to church, watching TV, crafting, and going to the lake. Dr. Conway is a specialist in internal medicine and focuses on the prevention, diagnosis, and treatment of adult diseases. She graduated magna cum laude from Xavier University in Cincinnati, Ohio with a bachelor’s degree in biology in 2011. Dr. Conway earned her medical degree from The University of Cincinnati College of Medicine in 2015 and completed her residency in internal medicine at The Christ Hospital in 2018.

 

Dr. Conway’s interests in preventative medicine and the management of chronic disease processes make her an excellent fit for FPA. She also has an interest in primary care dermatology. She aims to help adults take control of their own health to become and stay well. Dr. Conway is available for new patients over 18 years of age and routine office visits. She will be mainly located at the Harrodsburg Road office.

David P. Dubocq, MD

David P. Dubocq, MD

Dr. Dubocq is a native of New York state and came to Family Practice Associates of Lexington in 1998.  Dr. Dubocq believes “we need to work together as a team to help you and your family reach their highest level of health so you can enjoy the precious gift of life.”

Dr. Dubocq is also an artist and is responsible for many of the original works of art around the office.  He teaches elementary school children about sculpture and has made several collaborative works with them.  He is the co-founder of the Lexington Extraordinary Art Project (LEAP) which is a nonprofit organization dedicated to the enrichment of the Lexington community through public art.  Sculptures produced through this organization are located in the lobby and in front of FPA’s building  and other places around town.

Amanda R. Foxx, MD

Amanda R. Foxx, MD

Dr. Amanda Foxx grew up in Lexington, KY.  After graduating from Furman University in Greenville, SC, Dr. Amanda Foxx attended medical school at the University of Kentucky and graduated in 2011.  She then completed her residency in combined Internal Medicine and Pediatrics at UK in May 2015, where she was Chief Resident from 2014-2015.

Dr. Amanda Foxx has a special interest in preventative care and women’s health. She enjoys taking care of all patients from newborns to the elderly.  She is very excited to start her career at the same practice founded by her father.  Dr. Amanda Foxx is an excellent addition to FPA.

Dr. Ashley Rollins is originally from Lexington, Kentucky. She is married with one daughter. She enjoys spending time with her family, skiing, camping, and going to the lake. Dr. Rollins is a specialist in internal medicine and pediatrics, who can see patients of all ages, from newborns to the elderly.  She graduated magna cum laude from Transylvania University in Lexington with a bachelor’s degree in biology in 2009. Dr. Rollins earned her medical degree from The University of Kentucky College of Medicine in 2014 and completed her residency in internal medicine at the University of Kentucky Hospital in 2018.

 

Dr. Rollins can see all patients for any issues but has an interest in pediatrics, women’s health and preventative medicine.   She is excited to start her career at Family Practice Associates where she can care for entire families and develop long term relationships with her patients.  Dr. Rollins will be mainly located at our Hamburg office.

Whitney Adams, APRN

Whitney Adams is originally from Mt Olivet, KY but has resided in Lexington for many years. She is married with 2 children and enjoys reading and cooking. Whitney received her Bachelor’s Degree in Nursing (R.N.) in 2000 from Eastern Kentucky University and has experience in a variety of nursing roles including cardiology and pulmonary. Additionally, Whitney has had a long and distinguished career in the pharmaceutical industry with several companies.

After working as an R.N., Whitney decided to pursue a career as a Nurse Practitioner and attended Eastern Kentucky University. She graduated from EKU in May 2016 with a Master’s Degree in Nursing and is Board Certified as a Family Nurse Practitioner.

Whitney’s interests include women’s health as well as general adult and pediatric medicine. She can see patients of all ages and has begun her practice at Family Practice Associates of Lexington in September 2016.

Hannah M. Brugger, APRN

Hannah M. Brugger, APRN

Hannah Brugger is originally from Menifee County and grew up near Cave Run Lake, where she was involved in the community at a young age as a Presbyterian minister’s daughter.  She is married with a growing family and loves hiking, reading and travel.  Hannah received her Bachelor’s Degree in Political Science from Transylvania University before getting a Bachelor’s Degree in Nursing (R.N.) from the University of Kentucky.  After working for nearly 5 years as an RN in the hospital, Hannah decided to further her education and graduated as a Board Certified Family Nurse Practitioner in May 2017 from the University of Louisville.

 

Hannah’s experience and personality make her an excellent fit for FPA.  Hannah enjoys women’s health and preventive medicine issues but can see any patients 2 years of age and up.  Hannah will be performing Medicare Annual Wellness visits but she is available for new patient visits, well child and preventive adult visits, as well as routine office visits.

Lindsey Clickner, APRN

Lindsey Clickner was born and raised in Lexington and is married with 3 boys. She enjoys hiking, skiing and reading. Lindsey received her Bachelor’s Degree in Nursing (R.N.) in 2004 from Regis University in Colorado and has experience in a variety of nursing roles including critical and intensive care.

After working as an R.N., Lindsey decided to pursue a career as a Nurse Practitioner and attended Gardner Webb University in North Carolina. She graduated from Gardner Webb in May 2016 with a Master’s Degree in Nursing and is Board Certified as a Family Nurse Practitioner.

Lindsey’s interests include women’s health and will perform annual Medicare Wellness exams. She can see patients of all ages and has begun her practice at Family Practice Associates of Lexington in September of 2016.

January R. Hamby, APRN

January Hamby is originally from Leitchfield, KY.  She is married and enjoys traveling, scuba diving, and the outdoors.  January received her bachelor’s degree in nursing (R.N.) in 1999 from Eastern Kentucky University and has experience in a variety of nursing roles including emergency room, critical care and community nursing.  Additionally, January worked as adjunct faculty for 10 years with the Jefferson Community and Technical College Nursing program.

 

After working as an RN, January decided to further her education and obtained a Master’s Degree in Nursing Education at Walden University in August 2013.  Later, she decided to pursue a career as a Nurse Practitioner and attended Maryville University.  She graduated from Maryville University in May 2017 with a post Master’s Degree in Nursing and is Board Certified as an Adult Gerontological Nurse Practitioner.

 

January’s interests include geriatric health as well as general adult medicine.  She can see patients 18 years and older.
