Family Practice Associates was founded by Dr. Jeffrey Foxx in 1983 and has grown to one of the largest family practice groups in Central Kentucky. Today FPA is a group of primary care providers who are dedicated to giving family-centered care from birth to later years. FPA has over 20 primary providers including: physicians, nurse practitioners, PA, Psychiatric NPs, LMFT and physical therapists. With a professionally trained staff of CMA’s, receptionists, lab techs and others, FPA provides award winning care that has been recognized as a Patient-Centered Medical Home (PCMH) through the National Commission on Quality Assurance.
Patient-centered medical care is a model of care emphasizing coordination and communication. Its goals are to improve quality, enhance preventive services and to provide comprehensive chronic disease management. Introduced in 1967 by the American Academy of Pediatrics, the concept has broadened to include such descriptive phrases as accessible, continuous, comprehensive, family-oriented, coordinated, compassionate, and culturally effective. The PCMH model organizes care around patients, working in teams, and coordinating and tracking care over time.
Practices that are PCMHs become the “first contact” for patients and provide continuous and comprehensive care. Under the direction of the PCP, FPA’s patient care teams collectively take responsibility for on-going care of patients. The practice uses the “whole-person” approach which means patients actively participate in decision-making and give feedback to their provider about the care they are receiving.
Additionally, providers that are designated as PCMHs are experts at coordinating and integrating care through all levels of the health care delivery system including specialists, diagnostic services, hospitalization, home health, and long-term care. The model also takes into account the need for families to have accessible healthcare including same-day scheduling and extended hours. FPA Patients benefit from the FPA being an early adopter of health information technology.
