Family Practice Associates was founded by Dr. Jeffrey Foxx in 1983 and has grown to one of the largest family practice groups in Central Kentucky. Today FPA is a group of primary care providers who are dedicated to giving family-centered care from birth to later years. FPA has 14 primary providers including: physicians, nurse practitioners and a marriage and family therapist. With a professionally trained staff of CMA’s, receptionists, lab techs and others, FPA provides award winning care that has been recognized as a Patient-Centered Medical Home (PCMH) through the National Commission on Quality Assurance.
Patient-centered medical care is a model of care emphasizing coordination and communication. Its goals are to improve quality, enhance preventive services and to provide comprehensive chronic disease management. Introduced in 1967 by the American Academy of Pediatrics, the concept has broadened to include such descriptive phrases as accessible, continuous, comprehensive, family-oriented, coordinated, compassionate and culturally effective. The PCMH model organizes care around patients, working in teams and coordinating and tracking care over time.
Practices that are PCMHs become the “first contact” for patients and provide continuous and comprehensive care. Under the direction of the PCP, FPA’s patient care teams collectively take responsibility for on-going care of patients. The practice uses the “whole-person” approach which means patients actively participate in decision-making and give feedback to their provider about the care they are receiving.
Additionally, providers that are designated as PCMHs are experts at coordinating and integrating care through all levels of the health care delivery system including specialists, diagnostic services, hospitalization, home health and long-term care. The model also takes into account the need for families to have accessible healthcare including same-day scheduling and extended hours. FPA is working diligently to offer Central Kentucky families

FPA Patients benefit from the FPA being an early adopter of health information technology. In addition to FPA having an improved practice website, the practice has integrated a patient portal and electronic health records (EHR) into the services it provides to families. All of these forms of technology help patients receive optimal patient care, communication and patient education.
Whitney Adams is originally from Mt Olivet, KY but has resided in Lexington for many years. She is married with 2 children and enjoys reading and cooking. Whitney received her Bachelor’s Degree in Nursing (R.N.) in 2000 from Eastern Kentucky University and has experience in a variety of nursing roles including cardiology and pulmonary. Additionally, Whitney has had a long and distinguished career in the pharmaceutical industry with several companies.
After working as an R.N., Whitney decided to pursue a career as a Nurse Practitioner and attended Eastern Kentucky University. She graduated from EKU in May 2016 with a Master’s Degree in Nursing and is Board Certified as a Family Nurse Practitioner.
Whitney’s interests include women’s health as well as general adult and pediatric medicine. She can see patients of all ages and has begun her practice at Family Practice Associates of Lexington in September 2016.
Dr. Johnson completed his family practice residency at the University of Kentucky in November 2004, joining Family Practice Associates of Lexington in December 2004. Dr. Johnson’s particular interests include chronic disease management, pediatrics, and aviation medicine.
A Covington, KY native, Dr. Foxx founded Family Practice Associates of Lexington in 1983. Dr. Foxx holds the belief that “God is first, family is second, and medicine is third.”
Todd Martin is a native of Northern Kentucky where he graduated from Northern Kentucky University in 1990 and completed his Master’s Degree in 1999, receiving honors in both programs. He is Board Certified as a Family Nurse Practitioner and has worked in Emergency Medicine and in Family Practice. Todd has lived in Lexington with his wife, Lori, for the last 13 years, and is an avid fly fisherman and outdoor enthusiast. He joined Family Practice Associates of Lexington in 2013 and “believes in treating not just the disease, but the patient, as a complete human being”.
Dr. Monohan is board certified in both Internal Medicine and Pediatrics. She is a native of Winchester, KY and joined Family Practice Associates of Lexington in 2011.
A native of Beattyville, Kentucky, Mr. Kincaid has lived in Lexington for nearly twenty years. He believes his job as a marriage and family therapist is to “help people find and act on the truth.”
A native of Ashland, Kentucky, Dr. Henkel joined the Family Practice Associates of Lexington in 1996. Dr. Henkel’s goal as your family doctor is “to help you and your family maintain outstanding health and lead full, eventful lives.”
Lindsey Clickner was born and raised in Lexington and is married with 3 boys. She enjoys hiking, skiing and reading. Lindsey received her Bachelor’s Degree in Nursing (R.N.) in 2004 from Regis University in Colorado and has experience in a variety of nursing roles including critical and intensive care.
After working as an R.N., Lindsey decided to pursue a career as a Nurse Practitioner and attended Gardner Webb University in North Carolina. She graduated from Gardner Webb in May 2016 with a Master’s Degree in Nursing and is Board Certified as a Family Nurse Practitioner.
Lindsey’s interests include women’s health and will perform annual Medicare Wellness exams. She can see patients of all ages and has begun her practice at Family Practice Associates of Lexington in September of 2016.
A Louisville native, Dr. Applegate joined Family Practice Associates of Lexington in 1987. Dr. Applegate’s objective is “to have a helpful and rewarding doctor-patient relationship that results in a healthier you.”
Dr. Gerhardstein is a native of Fort Thomas, Kentucky and joined Family Practice Associates of Lexington in 2003. Dr. Gerhardstein shares Nietzche’s philosophy, “that which does not kill us only makes us stronger”.
A native of Louisville, Kentucky, Dr. Reesor joined Family Practice Associates in 2001 from a solo family practice in Lexington. Dr. Reesor’s goal is “to help patients live a long, healthy and prosperous life.“
Dr. Hayslip is a native of Ohio and moved to Kentucky from South Carolina with her family, joining Family Practice Associates of Lexington in 2007. Dr. Hayslip’s goal as your family physician is to “help you feel better and stay healthy.”
Dr. Dubocq is a native of New York state and came to Family Practice Associates of Lexington in 1998. Dr. Dubocq believes “we need to work together as a team to help you and your family reach their highest level of health so you can enjoy the precious gift of life.”
Dr. Dubocq is also an artist and is responsible for many of the original works of art around the office. He teaches elementary school children about sculpture and has made several collaborative works with them. He is the co-founder of the Lexington Extraordinary Art Project (LEAP) which is a nonprofit organization dedicated to the enrichment of the Lexington community through public art. Sculptures produced through this organization are located in the lobby and in front of FPA’s building and other places around town.
Cheryl Lewis is a longtime resident of Lexington and joined Family Practice Associates in 2012. She graduated from the University of Kentucky with a Bachelor of Science in Nursing in 1998, and then completed the Family Nurse Practitioner Program in 1999. Cheryl’s goal as your provider is “to continuously improve your health and well-being through honest, respectful, and compassionate care”.
Amber Ballard is a native of Beattyville, KY and currently lives in Lexington, KY with her husband and two children. Amber has practiced in Central Family Practice Associates of Lexington and has experience in Family Practice and Urgent Care. Her interests include women’s health and pediatrics. She joined FPA in September of 2014.
Dr. Amanda Foxx grew up in Lexington, KY. After graduating from Furman University in Greenville, SC, Dr. Amanda Foxx attended medical school at the University of Kentucky and graduated in 2011. She then completed her residency in combined Internal Medicine and Pediatrics at UK in May 2015, where she was Chief Resident from 2014-2015.
Dr. Amanda Foxx has a special interest in preventative care and women’s health. She enjoys taking care of all patients from newborns to the elderly. She is very excited to start her career at the same practice founded by her father. Dr. Amanda Foxx is an excellent addition to FPA.
