Family Practice Associates of Lexington (FPA) was founded by Dr. Jeffrey Foxx in 1983 and has since become one of the largest family practice groups in Central Kentucky. With over 20 primary care providers, including physicians, nurse practitioners, and physician assistants, FPA is dedicated to providing family-centered care from birth to later years. Recognized as a Patient-Centered Medical Home (PCMH) by the National Commission on Quality Assurance, FPA's professionally trained staff ensures award-winning care focusing on coordination, communication, and comprehensive disease management.
The PCMH model emphasizes accessibility, continuity, and comprehensive care, with patients actively involved in decision-making. FPA's patient care teams, under the direction of the primary care provider (PCP), collectively take responsibility for ongoing patient care. Through integration with health information technology, including a patient portal and electronic health records (EHR), FPA enhances patient care, communication, and education. By offering extended hours and same-day scheduling, FPA aims to provide ultimate convenience to simplify managing your health.

Dr. Applegate joined FPA in 1987 and is originally from Louisville, KY. He earned his medical degree at the University of Kentucky in 1984. He completed his residency at the University of Cincinnati and went on to complete his fellowship at the American Academy of Family Physicians in 1985. Dr. Applegate specializes in family medicine and can see patients of all ages.
For fun, Dr. Applegate enjoys bicycling, gardening, and traveling.
Originally from Maysville, KY, Dr. Conway joined FPA in 2018. Dr. Conway earned her medical degree from The University of Cincinnati College of Medicine in 2015 and completed her residency in internal medicine at The Christ Hospital in 2018. Her clinical interests include preventative medicine, management of chronic diseases, and primary care dermatology. Dr. Conway can see patients who are 18 years old and older.
In her free time, Dr. Conway enjoys spending time with her family, going to church, watching TV, crafting, and going to the lake.
Dr. Farmer joined FPA in 2021 and is a native of Lexington, KY. Dr. Farmer graduated from medical school at the University of Kentucky in 2005 and completed her residency in Internal Medicine in 2008. While at UK, she received numerous awards and recognitions, including Beale Primary Care Scholarship and Primary Care Resident Award. Dr. Farmer is trained in all aspects of internal medicine and its specialties, managing both acute and chronic conditions. Her clinical interests include Women’s Health, preventative medicine, and chronic disease management. Dr. Farmer can see patients who are 18 years old and older.
For fun, Dr. Farmer enjoys boating, snow skiing, and gardening.
Dr. Foxx is originally from Lexington, KY, and joined FPA in 2015. Dr. Foxx attended medical school at the University of Kentucky and graduated in 2011. She went on to complete a dual residency in Internal Medicine and Pediatrics at UK in 2015, where she was Chief Resident. Dr. Amanda Foxx has a special interest in preventative care and women’s health. She enjoys taking care of all patients from newborns to the elderly.
During her free time, Dr. Foxx enjoys spending time with family, reading, playing piano, and going to the lake.
Dr. Gerhardstein joined FPA in 2003 and is originally from Fort Thomas, KY. He graduated from medical school at the University of Kentucky in 1987. He completed his residency at the University of Kentucky and went on to complete a fellowship at the American Academy of Family Physicians in 1999. Dr. Gerhardstein specializes in all aspects of family medicine for patients of all ages.
For fun, Dr. Gerhardstein enjoys boating, hiking, travel, and biking.
Originally from Ashland, KY, Dr. Henkel joined FPA in 1996. Dr. Henkel attended medical school at the University of Kentucky in 1992 and completed her internship in Internal Medicine and residency in Family Medicine at the University of Kentucky in 1995. Dr. Henkel specializes in all aspects of family medicine for patients 6 years old and older.
During her free time, Dr. Henkel enjoys biking, Pilates, and skiing.
A native of Winchester, KY, Dr. Monohan has been with FPA since 2011. Dr. Monohan attended medical school at the University of Kentucky in 2000 and completed her residency at Indiana University in 2004. Dr. Monohan specializes in Internal Medicine and Pediatrics and can see patients of all ages.
Dr. Monohan resides in Lexington with her husband and her 3 children. Most of her free time is spent cheering on her children in their many sporting activities, including track, cross country, and gymnastics. Dr. Monohan’s other hobbies include boating and watching UK sports.
Dr. O’Leary graduated from medical school at Ohio State University before moving to Kentucky, where she completed a dual residency in Internal Medicine and Pediatrics at the University of Kentucky in 2016. Prior to joining FPA in 2023, she practiced primary care in the Lexington area. Dr. O’Leary enjoys caring for families from newborns and first-time parents to the elderly and people with chronic medical conditions.
Outside of work Dr. O’Leary enjoys being home with her husband and raising their two sons.
Dr. Reesor joined FPA in 2001 and is originally from Louisville, KY. He attended medical school at the University of Kentucky and completed his residency at the Memorial Medical Center in Savannah, GA. Dr. Reesor specializes in family medicine for patients who are 2 years old or older.
For fun, Dr. Reesor enjoys fishing and golfing.
Dr. Rollins joined FPA in 2018 and is originally from Lexington, KY. Dr. Rollins earned her medical degree from The University of Kentucky in 2014 and completed her residency in Internal Medicine at the University of Kentucky Hospital in 2018. Dr. Rollins specializes in Internal Medicine and Pediatrics. She also has an interest in women’s health and preventative medicine. Dr. Rollins enjoys taking care of all patients, from newborns to the elderly.
In her free time, Dr. Rollins enjoys spending time with her family, skiing, camping, and going to the lake.
Dr. Rossi graduated from medical school at the University of Kentucky in 2017. He went on to complete his residency in Family Medicine and joined FPA in 2020. While at UK, he received numerous awards and recognitions, including the Otis Singletary Scholarship. Dr. Rossi is trained in all aspects of family medicine and primary care, including management of acute & chronic conditions for patients of all ages.
For fun, Dr. Rossi enjoys hiking, making ceramics, and playing video games.
Andrea Bigelow obtained her master’s degree in nursing at Chamberlain University and became a Board-Certified Family Nurse Practitioner in 2020. Prior to joining FPA in 2022, Bigelow worked as a Neonatal ICU Registered Nurse for nearly 8 years at both Cincinnati Children’s Hospital and University of Kentucky Children’s Hospital. Bigelow specializes in pediatrics, women’s health, urgent care, and preventative medicine for patients of all ages.
During her free time, she loves boating, hiking, reading, and crafting.
Lindsey Clickner graduated from Gardner Webb in May 2016 with a master’s degree in nursing and is Board-Certified as a Family Nurse Practitioner. Prior to joining FPA in 2016, Clickner worked as an RN in critical and intensive care units. Clickner can see patients who are 2 years old and older. Her clinical interests include women’s health and annual Medicare Wellness exams.
In her free time, she enjoys hiking, skiing, and reading.
Todd Martin graduated from Northern Kentucky University with a master’s degree in nursing and has been a Board-Certified Family Nurse Practitioner since 1999. Prior to joining FPA in 2013, he had extensive experience in emergency and family medicine. Martin specializes in all aspects of family medicine for patients of all ages.
In his free time, Martin is an avid fly fisherman and outdoor enthusiast.
Mary Oaks graduated from the University of Kentucky in 2018 with a master’s degree in physician assistant studies. Prior to joining FPA in 2019, Oaks had extensive experience working in the urgent care setting. She can see patients of all ages and is the main provider of acute care at the same-day clinic.
For fun, Oaks enjoys gardening and trying new cuisine.
Meagan Obst received her master’s degree in nursing at Chamberlain University. She became a Board-Certified Family Nurse Practitioner in 2019. Prior to joining FPA in 2023, Obst worked in the urgent care setting and family medicine. Obst enjoys seeing patients of all ages, and her clinical interests include pediatrics, women’s health, weight loss management, urgent care visits, and mental health management.
She enjoys hiking, sporting events, cooking, reading, and being outdoors with her family during her time off.
Shelby Riggs graduated as a Board-Certified Family Nurse Practitioner in 2017 from Indiana Wesleyan University. Prior to joining FPA in 2017, she worked as an RN for 10 years in pediatrics and endocrinology. Riggs enjoys women’s health and pediatric issues but can see patients of all ages.
In her free time, Riggs enjoys reading and crafting.
Amanda Shackelford obtained her master’s degree in nursing from Chamberlain University and became a Board-Certified Family Nurse Practitioner in 2022. Prior to joining FPA as an APRN in 2023, Shackelford worked as a CMA for 6 years at FPA. Her interests include women’s health, preventative medicine, and chronic disease management. Shackelford can see patients who are 2 years old and older.
During her free time, she loves to travel (especially to Disney World), spend time with family and friends, bake, and plan events.
Kristy Carter joined FPA in 2023 and is an advanced practice registered nurse, ANCC board certified as a family psychiatric mental health nurse practitioner. She earned her master’s degree at the University of Kentucky College of Nursing in 2011. In addition to her Bachelor of Nursing, she has completed a Bachelor of Science in Psychology from the University of Kentucky, at which time she was also a member of the Psi Chi International Honor Society in Psychology. She practiced nursing for several years on a neurological/stroke/telemetry unit at Saint Joseph Hospital, where she received the DAISY Award for Extraordinary Nurses. She is currently a member of the American Psychiatric Nurses Association and the Neuroscience Education Institute. Kristy sees a variety of ages, from children five years old to geriatric, and provides medication management for a variety of mental health conditions, including addiction, anger management, anxiety, mood disorders (depression and bipolar), obsessive-compulsive disorder, psychosis, and trauma/PTSD.
For fun, Kristy enjoys hiking, spending time with her dog, and cooking.
Rae Holliday is a Licensed Marriage and Family Therapist. She earned her bachelor’s degree from Wake Forest University and her master’s from East Carolina University. Prior to joining FPA in 2023, Rae primarily served adolescents and families experiencing psychiatric crises during inpatient hospitalizations. Rae’s clinical interests include sexuality and gender identity, family conflict, communication, depression and anxiety, life transitions, and first responder/military. Rae sees patients who are 12 years old and older.
In her free time, Rae enjoys reading, being outside, hanging out with her family, and practicing yoga.
Latoya Lee is an Advanced Practice Registered Nurse, ANCC Board Certified as a Family Psychiatric Mental Health Nurse Practitioner. She earned her Doctoral degree from the University of Kentucky College of Nursing in 2017 and joined FPA in 2020. Dr. Lee enjoys seeing all lifespans, from children 5 years old to geriatric patients. She provides medication management for the following mental health disorders: ADHD, Mood Disorders: Depression & Bipolar, Psychosis, Personality Disorder, Addiction, Obsessive Compulsive Disorder, Anxiety, anger management, and Trauma/PTSD.
For fun, Dr. Lee enjoys being a football mom, cooking, and, of course, shopping.
