The COVID-19 MODERNA vaccine is now available @ FPA for patients 18 years of age & older! We have a limited supply. Getting your COVID-19 shot is easy, quick, & safe. FPA has a special SATURDAY COVID VACCINE CLINIC scheduled for Sat MAY 15th from 9:00AM – 12:00PM at our HAMBURG OFFICE located at 1775 Alysheba Way, Ste 201, Lexington KY 40509. Please use our Online Self-Scheduling option to schedule an appointment. You may also call office at 859-278-5007 or send us a secure web message from your FMH patient portal. If you have already received your first dose from another source (pharmacy, health dept) then please schedule your 2nd dose with that same source.
We appreciate everyone’s patience as we work hard towards vaccinating as many of our patients as possible and try to limit waste of our vaccine supply.
ALL vaccines will be provided by appointment only. No walk-ins accepted. First come, first serve for vaccinations based off availability & inventory. If you have an upcoming appointment with your provider, please let them know of your interest to get vaccinated.

What to Expect at Your Appointment to Get Vaccinated for COVID-19
You must Pre-register for your appointment once self-scheduled.You need to wear mask that covers your nose & mouth during the appointment.
On day of appointment upon arrival, please wait in your car. You will receive a text from FPA about 15 minutes prior to your appointment time & reply “HERE” to wait for more instructions. If you don’t receive a text & it is your appointment time, please come on into front lobby/check-in.
You will receive a vaccination card or printout that tells you what COVID-19 vaccine you received, the date you received it, & where you received it.You will receive an electronic version of a EUA fact sheet that tells you more about the specific COVID-19 vaccine you are being offered to help you understand the risks & benefits of receiving that specific vaccine during your pre-registration.
You will answer some screening questions & sign a consent form for the COVID19 vaccine during your pre-registration.
All people who get a COVID-19 vaccine should be monitored on-site for 15 minutes to observe for any adverse reactions.
You will be scheduled for your 2nd dose in 28 days from your 1st dose by a member of staff while you wait.
With mostCOVID-19 vaccines, you will need two shots in order for them to work. Get the second shot even if you have side effects after the first one unless a vaccination provider tells you not to get a second shot. It takes time for your body to build protection after any vaccination. COVID-19 vaccines that require 2 shots may not protect you until a week or two after your second shot.
