FAMILY PRACTICE ASSOCIATES OF LEXINGTON, PSC EARNS NATIONAL RECOGNITION FOR PATIENT-CENTERED CARE
WASHINGTON, DC – TheNational Committee for Quality Assurance (NCQA) announced today that FAMILY PRACTICE ASSOCIATES of Lexington, KY has received NCQA Recognition as a Patient Centered Medical Home for using evidence-based, patient-centered processes that focus on highly coordinated care and long-term, collaborative relationships.
The NCQA Patient-Centered MedicalHome program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP), American Osteopathic Association, and others. NCQA standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement.
